Patient Centered Medical Home

 

As Internal Medicine providers, we feel that every person should have a “Personal Medical Home,” that serves as the focal point through which they receive acute care, such as for injuries and illnesses, chronic care for longer term medical conditions such as high blood pressure and diabetes, and also preventive medical services, to minimize the need for acute and chronic care.  Your Medical Home should provide you with access to your Physician, your medical records and the medical information you need to make decisions on your health.  Our primary focus is improved patient outcomes and patient-centered care.  To demonstrate our commitment to this philosophy, Chesapeake Internists is seeking Patient Centered Medical Home (PCMH) recognition status through the National Committee for Quality Assurance, a non-profit organization dedicated to improving health care quality. 

 

What to expect from a Patient Centered Medical Home:

 

Improved Accessibility    You will have an on-going relationship with a personal provider and will have a care team which will work together with you to coordinate your care. We attempt to be more accessible by saving some same day appointments for acute visits in order to see patients in a timely manner for more urgent needs.  We continue to maintain a 24 hour call system for urgent needs outside of office hours but appreciate your cooperation in calling during office hours for routine issues.  You also now have access to your triage nurse/medical assistant via email through our secure patient portal.

 

Comprehensive Care   We attempt to make sure that your comprehensive medical needs are met.  This includes addressing your physical, mental and behavioral health needs.  We will also coordinate your care with your specialists and provide follow up care from any hospitalizations or emergency room visits.  In order to effectively do this, it is important for you to provide us with your medical history, medications and information about any care received outside of our practice, to include visits to specialists, recent testing, hospitalizations, urgent care or emergency department visits. 

 

You are at the center of your care.  We make every attempt to keep you apprised of your condition and those things that affect it.  You are involved in the decisions regarding your treatment options.  We provide care in a compassionate and considerate manner.

 

Scientifically valid   We will attempt to take advantage of evidenced based medicine in providing you with up to date recommendations and treatments to keep you healthy.  We also realize that there is still an “art” to medicine and care often needs to be personalized to the individual.  We will help you play an active role in your care by also supporting self-management.
